Here is what decades behind a pharmacy counter taught us about men and continence products: most men would rather leak than wear something that feels like a nappy. So they fold toilet paper into their underwear, avoid long drives, quietly stop going to golf, and tell absolutely no one. If that is you or the man you care for, this guide is the conversation men rarely get to have: what the male-specific products actually are, why they feel nothing like what you are dreading, and exactly what to buy first.

And before anything else: leakage after prostate surgery, with an enlarged prostate, or with age is common, manageable and nothing to be ashamed of. Around one in four men over 65 experiences it. The products finally caught up. The stigma just has not, so let us get practical.

Why the supermarket fails men specifically

Most continence products on general shelves are unisex in name and female-patterned in fact: absorbency zones in the wrong place for male anatomy, shapes that bunch, sizing that assumes hips. Men try one, it leaks or feels awful, and they conclude the whole category is not for them. The actual conclusion should be: wrong product, not wrong category. Male-specific designs put the absorbency forward, shape to the body, and cup rather than pad. The difference in daily life is enormous.

The four product levels, in plain English

Level 1: Guards and shields, for drips and dribbles. A slim, contoured pad that cups the front and sticks inside your normal underwear. What to buy first: the two-product start

Sizing: waist measurement for pants, and honesty for absorbency. Buying a level down out of optimism is the second most common mistake, and it shows up as 3am sheet changes. If you are between levels, take the higher one; unused capacity costs nothing.

After prostate surgery, specifically

Post-prostatectomy leakage is usually at its worst in the early weeks and improves over months, especially with pelvic floor physio, which we would encourage every man to ask his GP about. Will anyone be able to tell I am wearing protection?

With guards, protectors and modern male pants under normal clothing: no. The trunk-style pants in particular read as underwear to the eye and the ear. The products that rustled and bulked belong to a previous decade.

How many changes a day is normal?

Light leakage: two to three guard changes across a day is typical. If you are changing more often than that, the absorbency level is too low rather than your usage being wrong. Step up a level and the day gets simpler.

Should I see a doctor about leakage, or just manage it?

Both. Manage it with the right products today, and mention it to your GP, because causes range from prostate issues to medications and several are very treatable.
